The Report Needs to look like this.
Please Let me know which Tool should i Use to get this done..

Chandra.There are different ways to do this. Here are a couple:
* You can drop individual text boxes within a list.
* You can use a table, and add multiple detail rows to support vertical
arrangement of fields.
